If your company will be bidding local phone service, please identify the amount to be cost-allocated out for the ineligible services - DID lines, voice mail, caller ID, inside wire maintenance, 900 call blocking, or other bundled ineligible services.

Please provide pricing for 100Mb, 150Mb, 200Mb, and 250Mb bandwidths for each connection. One connection is to 308 Dozier Street, Callisburg, TX 76240; (940) 665; and one to 648 FM 3164, Gainesville, TX 76240; (940) 612. Please provide separate pricing for any installation, activation, or initial configuration. Please also provide separate pricing for any maintenance and technical support. CATEGORY TWO DISQUALIFICATION FACTORS:
· PROVIDERS WISHING TO BID ON THIS SINGLE PROJECT MUST ATTEND ONE OF TWO MANDATORY WALKTHROUGHS. THE FIRST WALKTHROUGH WILL BE FRIDAY, OCTOBER 30, 2015, FROM 9AM – 11AM CST AND THE SECOND WALKTHROUGH WILL BE FRIDAY, NOVEMBER 6, 2015, FROM 9AM – 11AM CST. BOTH DATES PROVIDERS SHOULD MEET AT THE HIGH SCHOOL, 308 DOZIER STREET, CALLISBURG, TX.
· BIDS RECEIVED FROM PROVIDERS WHO DID NOT ATTEND ONE OF THE MANDATORY WALKTHROUGHS WILL BE DISQUALIFIED.
· SERVICE PROVIDERS ARE REQUIRED TO BID THE ENTIRE HARDWARE PROJECT/UPGRADE. BIDS FOR INDIVIDUAL COMPONENTS OF THE PROJECT/UPGRADE WILL BE DISQUALIFIED.
· Vendors are required to submit bids via the template “Internal connections – Template V15 – 1” or “Basic Maintenance Template V15 – 1” for each individual school that is receiving equipment and a separate template for all shared equipment. Please name each template for the name of the school and project. Example: “Smith Elementary School Wireless Project” or “Wireless project shared equipment for all schools.” Vendors that do not submit bids in the required template format will be disqualified from the process.
CATEGORY TWO PROJECT DETAILS:
There is another document associated with this IFCB that is available for download, “Callisburg C2 List,” please download for further details. Please also separately identify any installation, activation, or initial configuration costs associate with any part of the project. We would also like for you to submit your bids in your normal format – a word/excel/PDF as you normally would. The winning vendor will be required to complete, for each school (and all shared equipment), the 2016 USAC C2 Templates when they become available. Once available, the templates will be due in a 14 day time period.
· Elementary School - MDF needs to be relocated (192 drops/cabling). Enterprise chassis switch (192 ports) in MDF and both IDFs. 3000va UPS w/network mgmt. in MDF and 1500va UPS w/network mgmt.. in both IDFs.
· Middle School: Replace two switches each in 2 IDFs (240 ports x 2); Aps and cabling will replace entire MS campus wireless. 1500va UPS w/network mgmt. in each IDF. Replace UPS’s for two additional IDFs
· High School: Enterprise chassis switch (480 ports) would replace four switches in the MDF; replace two switches in IDF (384 ports). Firewall in MDF. Wireless controller, APs and cabling will replace entire HS campus wireless. Symmetra in MDF and 2-1500va UPS w/network mgmt. In IDF’s.
