The following survey is designed to collect information for our participation in a federal grant program. We will be applying for federal assistance for various technology services under a program known as "E-rate" (Educational Rate). By filling out the following survey, you will help make sure that we receive the most funding possible under this program.

The E-rate program works by providing us with substantial discounts on eligible technology services. How much the government will pay for our services is determined by the income level of our students' families. Please fill out the following survey and return to us using the information below.
