IFCB Requirements :

·        All Questions and Bids must be submitted using the on-line IFCB system. If for some reason the system is down before the respective deadline, please email your bid to info@crwconsulting.com or fax it to 918.445.0049. Bids or questions submitted in this fashion will be disqualified if the on-line system is active at the time of submission.

·        Bidder must agree to participate in USF Program (AKA “E-rate”) for the corresponding funding year.

·        Please include the correct Service Provider Identification Number (SPIN) on your bid.

·        By submitting a bid, bidder certifies that the bidder does have a valid (non-red light status) SPIN for the E-rate program at the time of submission. Should the Applicant discover that the bidder is on red light status, or if the FCC classifies the bidder as on red-light status before work is performed and invoices are paid, the contract will be null and void and the applicant will have no payment obligations to the bidder.

·        Bidder is expected to provide the lowest corresponding price per E-rate rules.  See http://www.usac.org/sl/service-providers/step02/lowest-corresponding-price.aspx for details.

·        Contracts must not prohibit SPIN changes.

·        Bidder must agree to provide the Applicant the choice of discount methods (SPI or BEAR).

·        Bidder will be automatically disqualified if the District determines that the bidding company has offered any employee of the District any individual gift of more than $20 or gifts totaling more than $50 within a 12 month period.

·        All contracts awarded will be contingent upon E-rate funding and final board approval.  The applicant may choose to do all or part of the project upon funding notification.

·        All contracts awarded under this IFCB bidding process may be voluntarily renewed by the applicant, upon written notice to the provider, for five consecutive one year terms.

·        WEB HOSTING VENDORS ARE REQUIRED TO FILL OUT THE ATTACHED COVER SHEET FOR WEB HOSTING BIDS. WEB HOSTING BIDS SUBMITTED WITHOUT THAT COVER PAGE WILL BE DISQUALIFED. VENDORS NOT SUBMITTING WEB HOSTING BIDS ARE NOT REQUIRED TO USE THE COVER PAGE.

·        Maintenance bids listing only an hourly rate will be disqualified.  Vendors quoting an hourly rate are required to also a) confirm that you have the ability/expertise to maintain all of the equipment listed; and b) propose a number of hours at a particular monthly rate to properly maintain the entire list of equipment.  Bids that only contain hourly rates, without confirmation that the company can service the list of equipment, or without a monthly total, will be disqualified.

How do obtain more details regarding the bid requirements for the internal connections. The spreadsheet is very vague.

Answer:
 Vendors have two opportunities to view the facilities via the scheduled on-site meetings and If you have additional questions please post them to the web site using the “submit a question” button on this IFCB. If you have multiple questions, we suggest you combine them into one submission

 We would prefer bids for manufacturer specified on this IFCB. However, E-rate rules require applicants to accept bids for “functionally equivalent equipment.” Perth-Amboy may choose to award more points for vendors bidding the manufacturer requested.

Can Perth Amboy Public Schools provide their Avaya Sold #'s for their existing Avaya phone system at each school? Will Perth Amboy Public Schools submit a Letter of Agency to Avaya for a vendor that wants to bid on the Avaya phone system maintenance? This is needed to confirm the equipment inventory. How many SIP trunks are needed to replace PRI circuits? What model Avaya IP phone do you want quoted for the upgrade to VoIP? What model Extreme Switch is needed to be quoted?

 A list of the Avaya equipment has now been attached to this IFCB.

What model Avaya IP phone do you want quoted for the upgrade to VoIP? Avaya 9608 + Button Mod 12B (to replace Avaya 6424s), and Avaya 9608 (to replace Avaya 6408D+). Please include these on seperate bids as they are not e-rate eligible.
 
Our existing Extreme switches are 250e-48t and x460-24x. HOWEVER, whatever switches you quote, they will need to be POE.

Answer:
 Current calling plan is: AMERICAS CHOICE II 400 SHARE EMAIL & DATA+N&W+IN UNLIMITED

Shared allowance is 18,400
iOS for some and others are phone only.

The PerthAmboyAvayaList.xls document does not provide sufficient information to propose an upgrade of the Avaya PBX VoIP. Can we obtain the following reports from the Avaya PBX: list configuration-all, display system-parameters customer-options, display capacity, list cabinet, list media-gateway, list trunk? Is the current PBX maintenance contract supported by the manufacturer (Avaya)or from an authorized Avaya Business Partner? Since there is not sufficient information to provide an accurate quote for the Avaya VoIP upgrade, can the bid due be extend until January?

Answer:
 The upgrade to the phone system requested is only to provide SIP interfacing. We are not requesting an upgrade to the entire phone system.

 
Current PBX maintenance comes from a business partner.
 
The upgrade is just for SIP functionality, we do not plan to extend the bid deadline at this time.
