Leased Lit Fiber – Applicant currently has 300Mb bandwidth connection and is considering upgrading up to 5GB bandwidth. The connection is from 615 FM 1138 North, Nevada, TX 7517, (972) 843, to 801 Presidents Blvd, Lavon, TX, (972) 843. Please provide 1 year and 3 year pricing in 500Mb increments from 500Mb up to 5GB (500Mb, 1GB, 1.5GB, 2GB…5GB). Please include separate pricing for any installation, activation, and/or initial configuration costs. Please also include separate pricing for maintenance and technical support.
Internet Access – Minimum 100Mb bandwidth, maximum 300Mb bandwidth. The terminating address for this circuit is the NE Smith Elementary School, 801 Presidents Blvd, Lavon, TX 75166; (972) 843. Please provide pricing for 100Mb, 150Mb, 200Mb, 250Mb, and 300Mb bandwidths. E-rate rules require applicants to evaluate the cost of eligible goods and services, and to cost-allocate out the amount for ineligible services from their funding request. If your company will be bidding internet access service, please identify the amount to be cost-allocated out for the ineligible services – email service, web hosting, content filtering, or other bundled ineligible services.
Per USAC’s current guidance about applying for Internet access this applicant is checking the Leased Lit Fiber, Internet Access: ISP Service only and Internet Access and Transport Bundled.
DISQUALIFICATION FACTOR INTERNET ACCESS: Any and all installation charges must be identified on your bid. If you submit a bid indicating that installation charges may apply without listing those charges, we will consider your bid incomplete and disqualify your bid.

INTERNAL CONNECTIONS: There is another document associated with this IFCB available for download that details the equipment the applicant wishes to purchase. The document lists equipment that the application prefers – they will also accept functional equivalents of said items.
Project Narrative: The network switches are for our Campus Aggregation and Wireless network. The WS-C3850-24XS (or functional equivalent) should contain module C3850-NM-2-40G (or functional equivalent). All switches should have a redundant power supply option. All switches should contain stacking modules and cables appropriate for a 2U separation between switches. Please indicate on your proposal if you are a Cisco Certified Partner.
Compatibility Requirements: All switches must be compatible with Cisco 9K and Cisco 5520 wireless.
DISQUALIFICATION FACTOR INTERNAL CONNECTIONS: Vendors must complete and include the C2 Cover Page with their bid. Bids received without the Cover Page will be disqualified.
DISQUALIFICATION FACTOR INTERNAL CONNECTIONS: Service providers are required to bid the entire hardware project/upgrade. Bids for individual components of the project/upgrade will be disqualified.
DISQUALIFICATION FACTOR INTERNAL CONNECTIONS: It will be the bidder’s responsibility to provide documentation about the compatibility requirements listed in this IFCB. Vendors that submit bids without documentation demonstrating the required compatibility listed in this IFCB will have their bids disqualified, unless that vendor has bid the preferred make/model number listed.
DISQUALIFICATION FACTOR INTERNAL CONNECTIONS: Applicant will not accept bids for refurbished equipment. Any bids containing refurbished equipment will be disqualified.
