IFCB Requirements :
·        All Questions and Bids must be submitted using the on-line IFCB system. If for some reason the system is down before the respective deadline, please email your bid to info@crwconsulting.com or fax it to 918.445.0049. Bids or questions submitted in this fashion will be disqualified if the on-line system is active at the time of submission.
 
·        Bidder must agree to participate in USF Program (AKA “E-rate”) for the corresponding funding year.
 
·        Please include the correct Service Provider Identification Number (SPIN) on your bid.
 
·        By submitting a bid, bidder certifies that the bidder does have a valid (non-red light status) SPIN for the E-rate program at the time of submission. Should the Applicant discover that the bidder is on red light status, or if the FCC classifies the bidder as on red-light status before work is performed and invoices are paid, the contract will be null and void and the applicant will have no payment obligations to the bidder.
 
·        Bidder is expected to provide the lowest corresponding price per E-rate rules.  See http://www.usac.org/sl/service-providers/step02/lowest-corresponding-price.aspx for details.
 
·        Contracts must not prohibit SPIN changes.
 
·        Bidder must agree to provide the Applicant the choice of discount methods (SPI or BEAR).
 
·        Bidder will be automatically disqualified if the District determines that the bidding company has offered any employee of the District any individual gift of more than $20 or gifts totaling more than $50 within a 12 month period.
 
·        All contracts awarded will be contingent upon E-rate funding and final board approval.  The applicant may choose to do all or part of the project upon funding notification.
 
·        All contracts awarded under this IFCB bidding process may be voluntarily renewed by the applicant, upon written notice to the provider, for five consecutive one year terms.
 
·        Prices must be held firm for the duration of the associated E-rate Funding Year(s) or until all work associated with the project is complete (including any contract and USAC approved extensions). 

Services and Equipment Requested :
UPDATE 2/12/16: FOR THE LIT FIBER CONNECTION ON THIS IFCB WE WOULD LIKE TO SEE BIDS WITH A 3 OR 5 YEAR TERM, PLUS A FULL 10 YEAR TERM.

UPDATE 1/18/16 This 470 has now been posted and certified with the addition of the following Lit Fiber connection:

Lit Fiber Point to Point - Applicant would like a point to point connection from 3500 Neely to 615 W. Missouri at an 80 GB bandwidth. Strict SLA for low latency and high uptime. Please also include a bid for 2 x 50 GB. Please provide separate pricing for any installation, activation, or initial configuration.  Please also provide separate pricing for any maintenance and technical support. District would like a two year payment plan for any special construction charges.

 Leased Dark Fiber - Applicant would like to lease 6 pair of dark fiber between from 3500 Neely to 615 W. Missouri. Both in Midland, TX 79701. They are also seeking Modulating electronics to light dark fiber.
Dark Fiber Maintenance
Operations and Maintenance Practices:  The (School District) will require on-going maintenance and operations of the fiber for the term of the lease.  When pricing maintenance and operations, the respondent should include an overview of fiber maintenance practices including:

-Call before you dig locate services;
-Routine maintenance and inspection,
-Scheduled maintenance windows and scheduling practices for planned outages,
-Fiber monitoring including information on what fiber management software is used, what fiber monitoring system is used, and who performs the monitoring,
-Handling of unscheduled outages and customer problem reports?What service level agreement is included, and what alternative service levels may be available at additional cost,
-What agreements are in place with applicable utilities and utility contractors for emergency restoration,
-Repair of fiber breaks,
-Replacement of damaged fiber,
-Replacement of fiber which no longer meets specifications,
-Policies for customer notification regarding maintenance

When the District asks for 2x50Gb, is it looking for two paths @ 50Gb each or simply an aggregate of 100Gb of bandwidth?
